Here are 10 steps toward planning and implementing a finished website:
1) Create a Trello board
Trello is a fun, free way to break your planning and development process into small tasks that can be tracked.
2) Write your user stories
Here are some example user stories. These will guide how you think about your app's features and functionality. Note that they all follow a similar structure: as a <person> I can <do something>.
- as a logged-in user I can see the list of my to-do's.
- as a logged-in user I can add a new to-do.
- as a logged-in user I can delete a to-do (only my to-do's - not other users').
- as a logged-in user I can complete a to-do (only my to-do's - not other users').
- as an anonymous user, I can register for a new account, recover my password, or log in to the app with an existing account.
3) Create your use case model
This will help you visualize your user stories so you can better understand how to implement them.
4) Create an activity diagram
Your activity diagram will show the different paths your users can take through our app.
5) Create your mockups
Your mockups will show what your app should look like. It's much faster to iterate on a mockup than it is to do so on working code.
6) Choose the right technologies for our project
- MongoDB for your database
- Node.js and Express.js for implementing your API
- AngularJS, along with HTML and CSS (and Bootstrap) for your client-side application
- Mongoose to connect your application to MongoDB
7) Design your database schema

- What happens to the to-dos related to a user that deletes her account? When the user deletes her account the to-dos related to that user should also be deleted.
- No to-do can be added without being attached to a confirmed user.
- A to-do can only be deleted by its owner.
- No user can be added with an empty username or password.
- No to-do can be added with an empty task.
9) Design and test your API
We use a free product called Apiary to document our API.
10) Start writing code

Five Steps for going from Idea to Large Scale Web Application
The question is a bit vague on exactly what kind of website needs building. If it's a company brochure or a typical ecommerce system, those are well understood enough that Quincy's answer does the trick. If it's a web "application" like Facebook or AirBnB, you need a different plan:
Step 1) Write User Stories
A "user story" is just a discrete description of a task a person can do with your software. You phrase them like so, "as a <kind of person> I can <do a thing>". See Quincy Larson's answer for great examples of user stories.
User stories are fast to write and help clarify your thoughts and give you something you can discuss with others; people that will build it, people that will use it, and other people involved with the decisions (AKA, "stakeholders").
Step 2) Cut 50% and then cut another 50%
A complex system that works is invariably found to have evolved from a simple system that worked. A complex system designed from scratch never works and cannot be patched up to make it work. You have to start over with a working simple system. – John Gall
Most great ideas are ENORMOUS. Enormous websites cost tens to hundreds of millions of dollars to build and take years to build. Take Uber. They raised around around $300M in their first few rounds and probably spent all that cash building their platform. I suspect they've spent a lot more. You don't raise over $5B just for fun. They also started back in 2009, a full 6 years ago and they're still working on it. If they could have done it faster and cheaper, they would have. That's how much money and time it takes to build something significant.
If you force yourself to cut your list of user stories in half and keep only the most important items, and then cut it in half again, you'll have a highly focused list that's likely small enough to build effectively.
Try to release a site that does one thing fantastically and leave everything else out. Users will complain loudly about all the things it doesn't do, but if you pick the right things, they'll keep using it anyway.
Step 3) Build a Prototype
When you're building something new and interesting, there's a lot of risk:
Building a functioning, ugly, barely-gets-it-done prototype helps you mitigate those risks early. You can try a prototype with potential users, uncover and exorcise your technological boogy men, and show the bosses and investors something that actually works so they'll let you keep the doors open and the lights on.
Step 4) Polish and Release
Once you've got a prototype working that seems to resonate with a crowd of early adopters, then it's time to clean it up. Do a deep dive on user interface and graphic design. Fix all the bugs and get good code test coverage in place. Design and build your user onboarding systems.
Then release it. Get as many real people using it as possible. Test your marketing ideas and growth hacks and see if people love it, or leave it.
Step 5) Repeat
Building large scale software, as John Gall said, starts with taking something small that works, and iterating on it until it's something big that works.
That's how Facebook, AirBnB, Uber, Google, and most of the other dominate web apps did it. Learn from them. Start small, get it in the hands of real users early, and iterate your ass off. Depending on the scope of your project, you can DIY, or hire a designer. For more mission critical projects, or projects that are more complicated, you may want to hire an agency to help you go through the process. I've outlined the steps we take on a typical project below.
Determine your Users and Site Goal
I think the first step in any web design project is to determine who is going to use the site and what your goals are. It's very simple to design a site and get it online, but if it doesn't actually achieve your goals, what good is it?
Research the Market
Once you've mapped out your purpose and what kind of users the site will have, then you should build a plan. Research what other similar sites in the market space are doing. Figure out what kind of sites your target audience use. Understand the user pathways your audience are already used to.
Create Content and Navigation
Based on the strategy and user maps you've created, Then you should create a content and navigation plan. Create an outline that lists all of the content on the site and make sure that it speaks effectively to your needs. Make sure your navigation meets your user pathways intuitively and also makes sense for your SEO goals.
Design the Website
Now, we design. Create a wireframe mockup before you start building the final design. This will give you the overall sense of the site you need to evaluate whether the design is effective or not. Once you've done that evaluation, create a mockup that will be used to create the site.

Different kinds of sites will need different technologies. If it's a simple site, you might just use straight HTML / CSS. If it's for a client that needs to update it themselves, or you have a lot of content to manage, a CMS (content management system) like Wordpress may be appropriate. Optimize your Site.
Once the site is completed, optimize it for speed and SEO. Make sure all of your images are compressed. Minify your HTML / CSS code. Setup server caching.
Quality Control
Make sure you test your site before you launch it. So many sites are launched before they have actually been tested and run through a proper QC checklist. Quality counts!
Launch your website
It's finally time to launch your website. From this point on, make sure you get feedback from people who belong to your target user group so that you can make sure you've achieved your design goals. Also it's now time to plan your marketing and how to build traffic to the site.

- Ensure you have a scalable hosting platform, preferable cloud platform, if you're anticipating a large crowd to come swarming or your website is hardware intensive
- Ensure your site is optimized for speed. Refer to this, 20 Quick Tips to Optimize Page Load Time
- Ensure you have analytics tool, like Google Analytics
- Ensure you have a bug tracking system to track or report bugs. Even after your site has been up, you'd still encounter bugs

This allows you to put more details into the concept without having to actually build the thing already. Try bouncing around the concept with some colleagues or friends. Apply a peer review orsmell test. Sometimes, being too close to the idea gives us tunnel vision.The assemble the resources necessary to execute the idea or outsource it.
If you do it yourself or hire a team to do it then make sure you divide the work up into the most logical pieces and reduce dependencies. Try to keep to a schedule. Web development renders itself well to agile development concepts and A/B testing. So you can execute a smell test occasionally.
If you outsource it then you can ask your contractor to give you a few moreconcept screens and storyboards to further vet the idea. Pick the option you like then have it built. Regularly check on the work and apply some quality assurance.

Hey I'll add my 2¢ - I've launched 3 publicly facing products/websites in the past few years.
My most recent is www.earpod.co - which allows you to listen to podcasts in your browser. I started working on it in April of this year.
Before that was www.popple.co - closed down now though. This was a recruitment analytics platform that plugged into companies existing websites.
My last perhaps almost worth mentioning is www.whereis.ie - a college project that allows users to list business' details. The core of the project was an augmented reality android application that shows business' direction (as the crow flies) & distance on your screen. This was in 2011.
Anyway, start with a blank page and draw boxes of the layout you think you want. Refine it as needed. Hack that together and use basic color scheme for differentiation (re-work later as needed).
Develop core functionality and get user feedback. Iterate this again and again. Measure all valuable metrics where possible.
Keep iterating, keep measuring, & most importantly keep your feedback iteration going too.

I believe this company was later bought by Google. (The book is well worth the read and it's also available via Audible). Anyway.. The point is that this company came up with the most minimum viable product possible and then brought it in front of an audience. This allowed them to get an idea of how users searched and how they responded to different results.
Where am I going with this? Building a website or product is going to take you some time...More than you expect...but it doesn't need to be complete before you put it in front of the world. It's an iterative process. Come up with a minimum viable product that encapsulates your value added proposition and get it in front of users. They may rip it apart, but don't be afraid to take some criticism and come out of it with a better product.. Rinse and repeat.

I'm proud of all of them, but I felt like other developers would look at the code and say "alright that's pretty good code". I built that product with other developers I had a lot of respect for and we truly built something great... But we waited a year before customers ever saw it and the business got impatient. That project was canned and buried. The 4 other projects are still alive and thriving. They all involved smaller and leaner teams of developers (only myself for two of them) than the project that was canned.

I see a lot of good answers here but I want to point out one thing I don't see yet. Start with the end result you desire, work backwards. Once you have done that get opinions from people using the website in a normal way. Improve what you have and retool for each section of your website.

I prefer just to do many iterations on working prototype and when structure is right just do real features and beauty/css. Drawing interfaces is stupid, they wont be same in result and iterations will be needed anyway. Also if you are intrested creating site without special feautres you may use ready solutions like wordpress or squarespace
